Instructions

This questionnaire is intended to help you and your supervisor evaluate how effective and satisfactory your CBT supervision is. Using this questionnaire as the basis for a collaborative conversation could help clarify any changes that you and your supervisor could make to get more out of supervision. Simply circle the number under the answer that best reflects how you feel about the way that you and your supervisor are working together. Then add up the totals for each column on both pages and record the final total.

This is not a validated clinical instrument and is only meant to be used to guide the progress of supervision and to head off any problems at an early stage so that you get the best out of it. The range of scores is between 0 and 96 with the higher the score the better the supervision is going. A score of less than 48 overall, or a score of less than 3 on any item, should be considered a potential source of concern and would benefit from being discussed in your next supervision session.
